| Abstract: | The COVID-19 pandemic has exacerbated public health challenges in dengue-endemic regions, such as Federal District (Brazil), including cases of SARS-CoV-2 and DENV coinfection. In Chapter I of this thesis, 13 cases of SARS-CoV-2/DENV coinfection, diagnosed between April and September 2020 at the University Hospital of Brasília (HUB), were described. Information was collected from hospital medical records on the most relevant clinical and laboratory findings, the diagnostic process, therapeutic interventions, as well as patient outcomes and follow-up. These data highlighted the difficulty in distinguishing between the two infections, which share common symptoms and laboratory characteristics, delaying the diagnosis of SARS-CoV-2/DENV coinfection and compromising clinical management. Clinical improvement was observed in all patients within 21 days, indicating no worsening of clinical outcomes due to the coinfection. In Chapter II, untargeted metabolomics was applied to identify metabolic differences between patients with dengue, SARS-CoV-2/DENV coinfection, and COVID-19 (severe and non-severe). PCA and PLS-DA analyses showed a clear separation between the groups, suggesting distinct metabolic alterations, with emphasis on 31 putative metabolites including amino acids, bile acids, and lipids such as PA 40:3 and LPC 0:0/16:0, glycerophospholipids involved in the inflammatory response. Coinfected patients presented a metabolic profile more similar to that of dengue and non-severe COVID-19 patients, suggesting that coinfection does not exacerbate the inflammatory response or worsen clinical outcomes. To the best of our knowledge, this is the first experimental study of SARS-CoV-2/DENV coinfection. Chapter III compiled articles developed during the doctoral research, addressing various aspects of COVID-19 in the Federal District, in the context of the partnership between HUB and the Graduate Program in Medical Sciences at the University of Brasília (UnB). The studies investigated inflammatory mediators, coagulation profiles, mental health of healthcare professionals, and coinfections such as COVID-19/leprosy. This work contributed to expanding knowledge about these diseases and coinfections, including understanding the metabolic alterations in SARS-CoV-2/DENV coinfection compared to isolated infections. |
