| Abstract: | Introduction: Breast cancer is a high prevalent and incident disease, and it is considered an important public health problem. Sarcopenic obesity (SO), a condition that combines traits of obesity and sarcopenia, is associated with a series of negative outcomes in the oncology field. However, only recently a specific consensus was proposed aiming the diagnosis of OS (ESPEN EASO consensus). Thus, there is a lack of studies that diagnoses OS with a standardized criteria and evaluates the association of this condition with important outcomes. Objective: Apply the new SO consensus in women with early breast cancer, before clinical treatment, and examine the association between SO, quality of life, cancer-related fatigue (CRF), and physical function. Methods: Body composition was assessed by DEXA, and muscle strength was assessed by handgrip strength (HGS) and by the 5 times sit to stand test (5 STS). Physical function was assessed using the timed up and go test (TUG) and the 6 meters walk test (6 MWT), while quality of life and CRF were assessed using the EORTC QLQ-C30 and B-CFS questionnaires, respectively. The ESPEN-EASO consensus was used to diagnose SO. Therefore, the participants were divided into three groups: Eutrophic, Obesity and Sarcopenic Obesity. Results: Fifty participants were included in the study (48,4 ± 10,1 years), among those, 9 (18%) were classified in the Eutrophic group, 30 (60%) in the Obesity group and 11 (22%) in the Sarcopenic Obesity group. Lower values in global health status/quality of life (P = 0.03) were observed in the Sarcopenic Obesity group (58.3 ± 17.1) compared to Eutrophic (83.3 ± 18.2). The physical domain of the EORTC QLQ-C30, was lower in the Obesity group (84.2 ± 16.6) than in the Eutrophic group (99.2 ± 2.2). Physical fatigue and total fatigue were higher in the Obesity (5.1 ± 5.4 and 20.3 ± 9.2) and Sarcopenic Obesity (5.3 ± 4.2 and 20.4 ± 7.4) groups when compared to the Eutrophic group (1.9 ± 3.3 and 14.3 ± 8.5) but did not reach statistical significance (P = 0.10 and P = 0.12, respectively). TUG performance was worse (P < 0.05) in the Sarcopenic Obesity group (7.6 ± 0.7) compared to the Eutrophic (6.6 ± 0.7) and Obesity (6.6 ± 0.9) groups. Variables related to obesity and SO were statistically correlated with some quality of life, CRF and physical function variables. Conclusion: SO, identified through the consensus proposed by ESPEN-EASO, is associated with worse physical function than obesity alone, while obesity and SO are associated with fatigue and quality of life. |
