| Abstract: | Introduction: The treatment of chronic injuries entails high costs for health systems, as well as reducing the life quality of people affected by them. Aiming at improving the treatment of chronic wounds, it is possible that the use of L-PRF provides a favorable clinical response, with greater precocity in tissue repair, since autologous platelet concentrates can potentiate the stimulation of the physiological healing process and assist in the regeneration of various tissues. Objective: Evaluate the effect of weekly topical application of Leukocyte- and Platelet-Rich Fibrin in the repair process of chronic wounds. Methodology: This is an experimental, controlled, randomized, descriptive, comparative and unblinded clinical study, carried out at the Outpatient Service of Nursing in Stomatherapy (SAEE) of the University Hospital of Brasília, UnB - DF. The study was composed of two groups, namely: Control group (n=15) and Intervention/L-PRF group (n=15). The qualitative and quantitative parameters of the wounds in both groups were obtained by the same observer, once a week and for three months. In the qualitative analysis, the exposed tissues (superficial and/or deep), their measurement (area, perimeter and edge of the wounds), the macroscopic characteristics of the wounds and the evolution of the outcome (healing of the wounds) were considered. Results: Data analysis showed that: 1) there was a predominance of women in both groups, control and L-PRF; 2) there were no differences between the groups in terms of age, body mass index, total number of wounds, etiology of lesions and presence of comorbidities; 3) conventional or L-PRF treatment, as well as treatment time, did not affect the macroscopic characteristics of the wounds regarding the presence and intensity of pain, odor, lesion edges, periwound area, exudate; 4) treatment with L-PRF increased the percentage of epithelialization of wounds at 4, 8 or 12 weeks of treatment and at 8 weeks for the control group; 5) the conventional treatment did not affect the granulation tissue, while in the group treated with L-PRF there was a higher percentage of granulation tissue at 12 weeks; 6) there was no difference in the wound’s area for individuals in the conventional treatment group, while for the group treated with L-PRF there was a reduction in the area of wounds after 4 or 8 weeks of treatment; 7) L-PRF increased the percentage of individuals who had a reduction in the wound area, but did not change the time for wound closure/healing up to 12 weeks of treatment. Conclusion: The set of results indicated that Leukocyte- and Platelet-Rich Fibrin favors the healing/repair process of chronic vascular lesions, anticipating healing. Because it is a low-cost treatment and potentially superior to conventional treatments, can reduce the pain of treated individuals with a consequent improvement in the quality of life of these patients. |
