| Abstract: | Introduction: Muscle strength is essential for both functional and athletic performance, being particularly relevant to shoulder stability, a highly mobile joint prone to injury. Isometric strength assessment of shoulder muscles is a valuable tool in clinical practice, contributing to diagnosis, treatment, and rehabilitation monitoring. Although isokinetic dynamometers are reliable, their high cost and low portability limit their use outside laboratory environments. As an alternative, handheld dynamometers have emerged as viable tools, with the Kinology device standing out—a fixed-frame system offering an innovative approach to isometric strength assessment. Objective: Evaluate the intraand inter-rater reliability of the Kinology dynamometer in measuring isometric muscle strength of the shoulder joint. Methods: This is an intra- and inter-rater reliability study (test and retest) involving two assessors. Volunteers completed a demographic questionnaire, underwent exclusion-based special tests, and subsequently participated in a shoulder strength assessment protocol using a Kinology load cell and a mobile app-based data collection platform. The statistical analyses included ICC, SEM, and MDC, evaluating all participants and stratifying them by sex across four shoulder movements: abduction, flexion, and external rotation (in neutral and abducted positions). Results: The sample consisted of healthy young adults, with n = 81 volunteers, including 40 males and 41 females, with a mean age of 21.70 ± 3.41 years. The overall results showed high to very high intra- and inter-rater reliability, with all ICC values above 0.85. Standard error of measurement (SEM) values were below 5.05%, and minimal detectable change (MDC) values were below 13.99%, indicating significant reliability outcomes. The lowest ICC value was 0.849 for the inter-rater external rotation in abduction in males, while the highest was 0.940 for inter-rater flexion in females. Sex-stratified analysis revealed minor variations, with slightly higher ICCs observed among women. These findings are consistent with the literature and indicate good reproducibility of the device, reinforcing its clinical application potential. The fixed structure contributes to greater standardization and reduces examiner influence, highlighting Kinology® as a viable alternative to handheld dynamometers. Conclusion: The Kinology® portable dynamometer demonstrated excellent intra- and inter-rater reliability for measuring isometric shoulder strength in healthy young adults. The sex-stratified analysis showed minor variations in ICC, SEM, and MDC values, all within excellent ranges. The results suggest that the device is suitable for research and clinical use in similar populations. Sex stratification improved methodological precision. Future studies should expand the application to clinical populations, older adults, and broader testing protocols. |
