| Abstract: | With the outbreak of the COVID-19 pandemic and the record of a high number of deaths since its beginning, access to vaccines, recognized as essential to the containing of the disease, assumed a prominent place on the countries' agenda. However, the pandemic exposed that this way of safeguarding life did not reach everyone equally, being deficient or non-existent in peripheral countries and poor populations, a situation that clashes with the idea that universal access to the vaccine is a human right and a duty of the State to provide it. This premise, however, appears to be separated from bioethical principles, mainly those found in Bioethics of Intervention (BI) and the Universal Declaration on Bioethics and Human Rights (UDBHR). This thesis has the general objective of studying the explicit and implicit obstacles to free, universal and timely access to vaccination during the COVID-19 pandemic in Brazil. And, as specific objectives, to analyze how the Brazilian government dealt with public health issues during that period; investigate the ethical implications related to the management of actions offered to the population; critically discuss, based on the articles and principles of the UDBHR and the epistemological foundations of BI, theoretical references for the ethical debate related to the vaccination process in the context of the pandemic; and, analyze possible practical measures related to a scientifically correct, ethically defensible and socially fair national vaccination policy for the entire population. Analyzing the national and international public context of vaccination, the study provides information on the relevance of the National Immunization Program (NIP), the Unified Health System (UHS) and the World Health Organization (WHO) for the preservation of lives and supply of vaccines during that pandemic. It also brings elements to show the connection, patented in that period, between susceptibility to the COVID-19 virus and the social and environmental conditions existing in Brazil. In this sense, it seeks to show that such conditions result both from the market economy system in force in the country and from the continuity of a coloniality of knowledge, power, and life, which maintains its position of dependence on other countries in the process of production of vaccines. Methodologically, a theoretical-discursive and critical study was developed that uses the elements “vaccine” and “bioethics” for its theoretical-conceptual foundation. As a result, this thesis reinforced that the prospects of a disastrous denialist management adopted in Brazil during the COVID-19 pandemic were observed in the following areas: economic, with the delay in purchasing vaccines; political, by provoking confrontations between federative entities; legal, due to disrespect for the rules established to prevent the spread of the disease; and psychological, through the propagation of false narratives. Such distortions supported the proposition that vaccination and health processes are complex because they involve different areas of human relations, a context in which, if economic interests overlap with human interests, great losses can be expected, such as the impracticability of free and universal access to health services, both in relation to prevention and treatment, resulting in a high number of deaths. By bringing to the forefront ethical aspects involved in the issue of the right to a vaccine using theoretical references proposed by BI and nine of the 28 articles of the UDBHR, the present study sought to consolidate the importance of disseminating that Declaration and some BI concepts as a path to possible solutions of controversies and to create positive contributions in the scenario of negative vaccination processes. |
