| Abstract: | Diabetes Mellitus (DM) is a chronic, non-transmissible metabolic disease, with heterogeneousetiopathogenesis, triggered by defects in insulin secretion and/or action, causing carbohydrate,fat, and protein metabolism disorders. It is considered a public health problemduetoitsprevalence, impact on morbidity and mortality, and its complications. The IDFregistered537million individuals with DM in 2021, with 80% of this population living in low- andmiddleincome countries. Brazil is the fourth country in the world in number of DMcases, and50%ofdiabetics are unaware of the diagnosis. Early diagnosis and initiation of treatment preventsanddelays acute or chronic complications, micro and macrovascular, renal, and neurological;reducing mortality and morbidity; and reducing the cost for families and the healthsystem. Thetime of DM diagnosis is associated with increased risk of developing diabetic foot disease,being one of the most frequent complications in these individuals. Health care spendingforDMsufferers is $727 billion dollars annually, measured by the IDF in 2021. The objectiveofthisstudy is to analyze the economic efficiency of technologies available for the treatment ofwounds and foot ulcers in patients with Type I and II diabetes mellitus. This researchwasconducted by means of a systematic review in which different sources were analyzedanddiscussed. As a result, 10 eligible articles were screened, 5 of which were RCTs,for thisstudyand in this analysis it was observed that telemedicine in the treatment of the diabeticfoot waseffective and generated significant savings to the health system; the human dehydratedamnionand chorion allograft showed better cost-utility when compared to the standardisolatedtreatment of the diabetic foot, generating savings. A significant economic and healthburdeninthe health area was observed in the analyzed studies, which highlightsthe urgencyof publicpolicies aimed at prevention. The need to identify the best health technologies andtheircost-effectiveness was also observed. Conclusion: Telemedicine proved to be efficient intherapidtriage of diabetic foot ulcers, was effective in the course of treatment, and provedtobeeconomically viable and less costly when compared to outpatient care. With regardtotreatment,the dehydrated human amnion and chorion allograft showed cost-utility comparedtotreatmentalone. Further economic studies and intervention projects are needed to insert these changesintothe health care system permanently improving the treatment and prognosis of patientswithdiabetic foot ulcer, and reducing the costs related to these individuals. |
