Abstract: Objectives: To analyze the offer and quality of breastfeeding promotion services in Primary Care (PC) in Brazil. Methods: Cross-sectional study using secondary data from the Program for Improving Access and Quality of Primary Care (PMAQ-AB) in 2014/2015. We investi-gated actions related to the promotion of the breastfeeding in the External Evaluation Modules of the Program, resulting in the use of the Teams Module and Users Module. The BP promo-tion practices evaluated correspond to the actions offered in prenatal care, puerperium and child care, totaling 13 variables reported by users (9,595) and 12 others by health teams (29,778). The chi-square test was used, with significance level ≤ 0.05. The quality of the PC services in the promotion to the breastfeeding was estimated from a General Quality Score (GQS), classified as high, medium or low adequacy. Results: It was observed a low execution of the actions offered in prenatal, puerperal and puericulture, being the actions less perceived by the user than declared by the teams. The main shortcomings were breast care, group offer-ings, guidance on pacifier and bottle feeding, home visits by Community Health Agents, and the knowledge and application of the Brazilian Standard for the Marketing of Foods for In-fants and Young Children, Nipples, Pacifiers and Bottles (NBCAL). Regarding the analysis of the performance of PC actions according to GQS, only 15.4% of the services were classified as high GQS, 81.2% medium GQS and 3.4% low GQS. The North region concentrates the highest proportion of services with low GQS, 6.8% and lower with high GQS, 6.7%, followed by the Central West region with 9.9% of services with high GQS and 4.2% with low GQS. Municipalities with more than 100 thousand inhabitants obtained more services with high GQS, 17.6%, the same occurs for the stratification of the Human Development Index (HDI), the higher the HDI, the higher the proportion of services with high GQS. Municipalities with lower coverage of the Family Health Strategy (FHS) had a higher proportion of services with high GQS. Conclusions: There is a low adequacy of the promo-tion practices of breastfeeding in PC when using the criteria evaluated by PMAQ-AB. This result points to the need for structuring interventions in the context of the promotion activities of the breastfeeding in , especially PC with the greater dissemination, NBCAL supervision and articulation with the spaces of management and decision of the National Health System, description of attributions of actions that should be carried out in the PC, intensify the actions of permanent education, promote the electronic registration of health information and finan-cial linkage as an incentive to carry out actions.
