| Abstract: | This study aimed to develop and verify the evidence of validity of an instrument to assess mobility in the practice of hippotherapy on the horse, as an effect of the hippotherapy treatment. The method was divided into three stages: 1. Survey of data on the effects of hippotherapy on different functionality outcomes and identification of gaps in the assessment process, through a systematic review and meta-analysis; 2. Methodological study for the development of an instrument to assess the mobility of the practitioner of hippotherapy on the horse and verification of validity evidence; 3. Methodological study to verify the validation of competition and responsiveness of the Mobility Assessment Scale for Hippotherapy. The collections of these studies were carried out from August 2018 to December 2019 at the National Association of Riding Therapy and some affiliated Riding Therapy centers in the five regions of Brazil. The sample of methodological studies consisted of children and adolescents with different health conditions. Data were transmitted and each stage of the study observed as statistical analyses: in the development of the scale, the Content Validity was used, with analyzes using the Content Validity Coefficient (CVC); for construct validity, an exploratory factor analysis, variables from the structural equation model and Cronbach's Alpha are used to analyze the internal consistency of the items; for intra- and inter- examiner reliability, the intraclass correlation coefficient. For an analysis of competition validity, the Pearson correlation coefficient (r) was used and for the responsiveness of the scale, the test selected for a repeated measures ANOVA with Sidak post-hoc. Data normality was analyzed using the Shapiro-Wilk test. These studies present a milestone in how to assess the effects of hippotherapy. The results of the systematic review, of 23 articles, showed evidence of positive effects of hippotherapy on exercise tolerance (p = 0.004) and quality of life (p < 0.0001), with high confidence business effects; mobility (p = 0.002), interactions and interpersonal opportunities (p < 0.0001), with moderate confidence. No studies were identified that verified the effects of hippotherapy on mobility on the horse, that is, expected changes in the ability to maintain and change position on the horse and lead the animal. The first methodological study resulted in the construction of the Hippotherapy Mobility Assessment Scale (EAMEQ), which showed good inter- examiner (intraclass correlation coefficient: 0.991-0.999) and intra-examiner (intraclass correlation coefficient: 0.997-1.0) reliability, with excellent internal consistency (Cronbach's α: 0.937-0.999). An exploratory factor analysis grouped the EAMEQ items into three domains into one factor. It is concluded that the developed instrument is theoretically consistent and can be considered adequate and reliable for use. The scale can contribute to the assessment of the mobility of the hippotherapy practitioner on the horse, in addition to standardizing practice and homogenizing outcomes. In the second methodological study, it was identified that an EAMEQ has good concurrent validity with the Gross Motor Function Measure (GMFM), r = 0.872 (p <0.0001), in addition to good responsiveness over 15 weeks and three measurements (in the third visit , in the ninth service and in the fifteenth service, F (2.0, 80.0) = 56.00, p <0.0001. Sidak's Post-hoc showed that treatment in hippotherapy differs at all evaluation times. It is concluded that the developed instrument has good validation evidence, all the tested psychometric properties were obtained from the evaluation, the mobility of the hippotherapy practitioner on the horse. |
