| Abstract: | Introduction: The prevalence of breastfeeding and exclusive breastfeeding in Brazil has increased since the 1970s, but according to the latest available data, there was a stabilization of these rates. Studies show a high frequency of consumption of milk other than maternal milk at very early ages, and show that only 63% of children between 6 and 12 months consume fruits and vegetables daily, and between 12 and 18 months of age this percentage drops to 57%. These data demonstrate the need for adequate guidance to caregivers of these children, a task usually performed by health professionals. To that end, food guides and institutional materials are often the materials chosen by professionals to seek knowledge in the field of food and nutrition. These materials, especially the guides, correspond to the translation of scientific knowledge in nutrition into public health orientations aimed at the family and health professionals and are recognized as an essential part of national food and nutrition policies. Objective: To evaluate the knowledge of health professionals in Brazil regarding the official recommendations on breastfeeding and complementary feeding. Methods: This is a descriptive and cross-sectional study, with a quantitative approach and a national scope, in which the survey method for knowledge, attitudes and practices (KAP) was used, with data collection performed through the application of an online questionnaire for 316 nurses, physicians and nutritionists, selected for convenience and recruited from the snowball methodology. For the evaluation of the knowledge, items were elaborated by consulting institutional materials of the Ministry of Health and the Brazilian Society of Pediatrics, as guides and manuals, containing official guidelines about breastfeeding and complementary feeding. Thus, phrases (recommendations/knowledge to be measured) were presented and the health professional had to express their degree of agreement with this knowledge on a five-point Likert scale - 1 total disagreement and 5 total agreement. For the data analysis, the mean and the frequency of coincident responses were calculated, considering as knowledge gap the items with scores between 2.5 and 3.5 and as consolidated knowledge those with more than 80% of disagreement or total agreement. For the discussion of the results, the knowledge was categorized according to the theoretical model of attributes, components and food markers. Data were analyzed using SPSS software and descriptive analyzes were performed. Results: As a result of these analyzes, of the 89 items studied, 22 items were considered knowledge gaps, being the item with more neutrality related to the vegetarian diet in the food introduction phase (M = 3.137). The gaps still appeared in relation to the themes: supply of specific food groups and/or nutrients, consistency of the preparations, timely age for food introduction and allergenic foods. In relation to the consolidated knowledge, only 13 items were considered consensuses, the majority dealing with the biological safety and the nutritional adequacy of the preparations. It should be noted that omissions were found regarding certain issues and also, divergences in the recommendations in the institutional materials consulted. Conclusions: Health professionals who work in the orientation of breastfeeding and the introduction of complementary feeding still have many doubts about certain themes. So, it seems to be necessary to revisit and constantly update the materials available to these professionals, with special attention to the topics still identified here as knowledge gaps or as missing in the official recommendations. Some strategies have already been adopted, such as the revision of the Food Guide for Children by the Ministry of Health and the publication of Guidelines and Consensus on specific themes by the Brazilian Society of Pediatrics. These actions help to improve health education and training processes and, consequently, the improvement of the recommendations provided by professionals to the community. |
