| Abstract: | Background: The request for tumor markers (TM) for neoplasm screening in clinical practice is common. Objectives: to investigate the frequency of request as well as the costs related to the inappropriate request for TM in a private self- funded health care. Method: the database of a private self-funded health care from 2010 to 2017 was used. Subjects aged ≥ 50 years old whose medical records were available and who had undergone the measurement of some TM in that period were included. It was considered “inappropriate examination” when the TM was used as a neoplasia screening, that is, when there was no previous diagnosis. Data on age, sex, the specialty of the requesting physician, and information on the costs of the tests were evaluated. Results: A total of 1,112 tests were performed in the period 2010 - 2017, accounting for an increase from 52 to 262 exams per year. The sample was composed mostly of females (69.5%) with a mean age of 59.4 ± 8.2 years. Most requests were inappropriate (87.8%). It was observed that the request for these tests had an impact of approximately 79.4% of the total health plan expenses with TM. Cardiologists were the specialty that most requested TM (23.9%), followed by general practitioners (22.7%) and gynecologists (19.2%). Conclusion: The percentage of inappropriate tests requested was quite high, resulting in high costs. Studies of this nature shall be taken into account by health managers; also, interventions so as to adapt the use of TM in clinical practice must be considered. |
