Abstract: Introduction: Yellow fever (YF) is an acute viral disease transmitted by vectors of the genera Aedes, Haemogogus and Sabethes, and caused by an RNA virus that belongs to the genus Flavivirus. The main prevention and control strategy is vaccination, which has been shown to be highly immunogenic. Since its introduction, among vaccinees only 12 suspected cases of the disease have been documented in the world. In Brazil, between the years of 2016 and 2019, a total of 61 confirmed cases of the disease had a history of previous vaccination. For this reason, to review the associated factors with vaccine failure is relevant to understand and better systematize the available evidence on this topic and to clarify possible factors associated with confirmed cases among vaccinees in Brazil. Objectives: To describe, evaluate and systematize the evidence in the literature about yellow fever vaccine (YFV) and associated factors. Method: A systematic review (SR) of epidemiological studies was carried out on the evidence of YFV failure and associated factors. The databases used were: PubMed, Cochrane CENTRAL, Embase, LILACS. The quality of observational studies was assessed using the Newcastle Ottawa Scale and experimental studies, the Cochrane Collaboration. This review followed in the footsteps of Preferred Reporting Items for Systematic Reviews and Meta-Analyzes. Results: A total of 1466 articles were identified after searching the databases and after applying the inclusion criteria and 77 articles were included in the qualitative analysis. Of these articles, 59.8% were analytical, 40.2% were descriptive. Most of the cohort studies related to the outcomes were of good quality and in clinical trials, concealment of allocation and blinding were the items that received the lowest evaluations. The SR indicated that the YFV produces protective immunity in different age groups (except children under two years old), different types of producers, in simultaneous administration with other vaccines, in fractional doses, in the presence of adverse events following-immunization, using prophylactic and immunosuppressive therapies, in chronic myeloid leukemia, hematopoietic stem cells transplants and eggs allergy. It failed to produce a protective response in pregnant women, children under two years of age, children with Kwashiorkor and time after vaccination. For HIV + individuals, the results were divergent. Conclusions: The results of this SR conclude that YFV has a high immunogenic capacity and few elements compromise its response and gave visibility to the factors associated with the failure of the vaccine and its findings may support the recommendations regarding the vaccination policy, prioritizing future researches and enhancing the vaccine credibility in the prevention of a serious disease such as YF.
