Abstract: The dissemination of carbapenemase-producing Klebsiella pneumoniae (KPC +) and methicillin-resistant Staphylococcus aureus (MRSA) strains has become a significant problem worldwide. One of the strategies developed to control such microorganisms has been the antibiotics therapy combination. In the present work we investigated the in vitro and in vivo efficacy of antimicrobials penicillin G (PNG), chloramphenicol (CAP), streptomycin (SMA) and cefotaxime (CFT) alone and in combination with clinical isolates of K. pneumoniae and S. aureus. Clinical isolates of K. pneumoniae KPC + and S. aureus MRSA were exposed to antimicrobials at varying concentrations for 24 h. Peer-to-peer combinations of PNG, CAP, SMA and CFT were tested to evaluate their possible interactions using the checkerboard method. Synergism was defined as IFIC ≤ 0.5, as well as the reduction of the initial bacterial load of 105 CFU.mL-1 after 24 h, from associations. Minimum inhibitory concentration (MIC) was determined by serial broth microdilution according to CLSI. The cytotoxic activity of free and combined antimicrobial was evaluated on RAW 264.7 monocytes, African green monkey kidney cells (Cercopithecus aethiops) and erythrocytes. The MIC values obtained for CFT, CAP, SMA and PNG were 32, 8, 4 e >512 μg.mL-1 against K. pneumoniae, and 256, 64, 4 e 32 μg.mL-1, respectively, against S. aureus. The combination of CFT + SMA was the most effective against K. pneumoniae, demonstrating a synergistic effect (FIC = 0.18). The combination of CFT + SMA, among the other combinations against K. pneumoniae, showed effectiveness with a synergistic effect of lower value (IFIC = 0.18), as well as PNG + SMA against MRSA (IFIC = 0.18). None of the free and combined antibiotics showed hemolytic and cytotoxic effects on RAW 264.7 and Vero cells. Based on the results obtained, the combination of CFT + SMA and PNG + SMA may be a therapeutic alternative in the treatment of infections caused by K. pneumoniae KPC + and MRSA, respectively.
