Abstract:	Since the decentralization management process of the Unified Health System in Brazil, the debate was focused on the capacity of counties to implement one of the largest public policies in the world. The present study aims to analyze the government capacity of municipal health secretary in counties of Bahia. This is a descriptive, cross-sectional, quantitative-qualitative study. A municipal health secretary representing a small county (size I) and a large one (size II) was selected as the key informant of the research, in each of the 9 health macroregions of the state of Bahia. A questionnaire subdivided in 03 Dimensions was used and to the obtained answers was applied a punctuation matrix that allowed the quantification for the classification and comparability of the performance of the municipalities as to the capacity for the development and implementation of their government projects. The results showed that, of the 15 participating counties, 12 presented moderate and 03 presented low capacity of government. The Dimension related to the Organizational Design of the Municipal Health Secretaries was the one in which the counties presented better performance, followed by the Dimensions referring to the Working Systems of the Secretaries and the Professional Trajectory of the Managers. The size I counties obtained a better overall performance in relation to those of size II. In the same way, counties located in more developed regions obtained better results. Considering that the Leaders' Expertise was the main barrier to the development of the capacity of government in the secretaries studied, the importance of the investment in training initiatives and institutional support is emphasized. It´s recommended to prioritize small counties, in addition to those located in regions of greater socioeconomic vulnerability level, in order to promote the qualification of municipal management and consequent strengthening of the Unified Health System.
