Abstract: Obesity is an emerging epidemic health problem once it increases several risks of chronic diseases. Reductions in calorie intake, lifestyle changes, and exercise are the main approaches to manage obesity and correlate disorders, but successfulness is not achieved by most patients. Alternate day-fasting (ADF) has been reported as an effective method of weight loss and as an alternative for Calorie Restriction (CR). Thus, the aim of this study is to analyze the alterations in body composition and biochemical exams caused by CR and ADF in overweight subjects. With a group of 20 overweight subjects, composed by men and women, with age between 25 and 51 years old, it was made CR and ADF for 4 weeks. CR group (n = 9) had an hypocaloric diet, with 25% of the energy needed restricted, in 6 meals/day, while the ADF group (n = 11) had an hypocaloric diet, with 75% of the energy needed restricted, in 6 meals/day, alternated with an ad libitum day. Nutritional analyzes were made based on the Brazilian Food Composition Table (TACO) and CalcNut® Platform, of the Nutrition Department of the University of Brasilia (UnB). Body composition was analyzed by the BodPod Gold Standard (Cosmed®, Itália) and InBody 770 (Teprel®, Portugal), tests of excellent replicability, safe, non-invasive, fast (5 minutes each), comfortable and with instantaneous result. Biochemical blood samples were collected and analyzed by qualified professionals at the specialized laboratory SABIN®, to promote more comfort and safety for subjects and professionals. To compare if variants were significantly different between groups and each group, it was made the t-test and ANOVA, and it was consider significantly different the results with “p < 0,05”. Baseline characteristics of each group demonstrated differences, considering age (p = 0) and BMI (p = 0). The average of weight (p = 0,77), total fat (p = 0,98) and lean mass (p = 0,44) alterations were similar between groups CR and ADF. Regarding visceral fat, the average reduction in CR group was more intense than in ADF group (p = 0,04). Both groups demonstrated a similar water balance (p = 0,39). Alterations in biochemical exams were also similar between groups: blood glucose (p = 0,87), total cholesterol (p = 0,74), triglycerides (p = 0,19), HDL cholesterol (p = 0,10), LDL cholesterol (p = 0,99), free-T3 (p = 0,11), TSH (p = 0,97), free-T4 (p = 0,10), cortisol (p = 0,10). According to the food consumption analyses, the medium consumption of total calories and kcal/Kg (p = 0,18 e p = 0,43, respectively), total carbohydrate and g/Kg (p = 0,12 e p = 0,08, respectively), protein total and g/Kg (p = 0,79 e p = 0,05, respectively) and total fat (p = 0,05) was not different between groups. Only the consumption of fat g/Kg was statistically different between groups (p = 0,0002). Although ADF and CR presents some 10 similar results, ADF diet can not be consider similar because it does not provide the ideal nutrition, with all nutrients adequate intake, and it can promote lean mass loss. This pilot study open doors for future studies, that can make these interventions for longer period and greater number of subjects. In addition, it is an experimental study that can not be used at clinical practice, since the ADF has not enough scientific background to justify its use with safety. Thus, long-term studies with similar objectives are needed o confirm our results and increase our knowledge about ADF.
