Abstract: Introduction: Breastfeeding in the first hour of life provides numerous benefits to the mother and the newborn. Health professionals should support the mother and children pair for the breastfeeding in the first hour to happen properly. Objectives: This study aimed to understand the factors associated with breastfeeding in the first hour of life in the Northeast of Brazil in 2010, and the Federal District in 2011. Methodology: Two studies were conducted: the first, from secondary database of Neonatal Call conducted in the Amazon and in the Northeast of Brazil. This study included 8,471 pairs mothers and children under one year of age, residents in the Northeast, who attended the national vaccination campaign in 2010; and the second, with the data obtained in the Neonatal Call the Federal District, it was considered the sample of 1,027 mothers and children peers who attended the second phase of the vaccination campaign against poliomyelitis in the Federal District, Brazil, in 2011. For both studies breastfeeding in the first hour of life was the dependent variable. The independent variables were: sociodemographic characteristics of the mother and assistance to prenatal care, childbirth, postpartum and child health. The independent variable: physical, verbal and neglect at delivery was evaluated only for the Federal District. The reasons of crude and adjusted prevalence were used as a measure of association and calculated by Poisson regression for the two studies. Results: In the Northeast, 62.4% of newborns were breastfed in the first hour of life. Mother and children without rooming-in after birth (PR = 0.13), women who made cesarean (PR = 0.86), who evaluated the quality of childbirth care as satisfactory, poor or very poor (PR = 0.91), who had inadequate prenatal care (PR = 0.92), who had no companion postpartum (PR = 0.92), who had no companion at delivery (PR = 0.93) and evaluate the quality assistance received during prenatal care as satisfactory, poor or very poor (PR = 0.93) were less likely to breastfeed in the first hour. The only maternal characteristic associated age below 20 years (PR = 0.91). In the Federal District the prevalence of breastfeeding in the first hour of life was 77.3%. Mother and children that did not stayed in rooming-in after birth (PR = 0.28), had not received prenatal care appropriately (PR = 0.72) and cesarean section (PR = 0.88) were factors that interfered negatively in breastfeeding in the first hour. Conclusion: Factors associated with breastfeeding in the first hour of life, both in the Federal District and in the Northeast of Brazil, were predominantly related to health services and practices of professionals, therefore can be modifiable.
