Abstract: Ibrutinib (IBR) is a tyrosine kinase inhibitor under investigation in preclinical and clinical settings as an alternative treatment for melanoma. However, the limited oral bioavailability of IBR and the need for high doses of the drug to kill melanoma cells are important challenges to be overcome before the clinical use of the drug for this purpose. Considering that melanoma is restricted to the skin in its early stages, topical application of IBR may constitute an effective route of administration with a lower risk of adverse effects. However, the development of topical formulations for the drug requires initial studies on IBR's toxicity and permeation profiles in human skin. In this study, we determined the toxicity and dermatokinetics of IBR using human primary cells and human organotypic skin explant cultures (hOSEC). After demonstrating that primary human fibroblasts and keratinocytes present IBR target genes, the cytotoxicity of the drug was determined using MTT and annexin V/PI assays. IBR skin toxicity was assessed using the TTC assay, and irritation potential was established using histological evaluation. Finally, the skin permeation of the IBR was evaluated ex vivo to determine the dermatokinetics of the drug. Our findings reveal that IBR exerts dose-dependent toxicity on skin cells. Keratinocytes showed IC50 in the same concentration range as melanoma cells, while fibroblasts showed a lower sensitivity to the drug. Topical application of the drug induced little skin irritation and toxicity, and the drug was shown to successfully permeate the stratum corneum and reach viable layers of the skin in therapeutic concentrations. Overall, our data encourage the topical application of IBR in the treatment of melanoma, paving the way for future studies on the topic.
