Abstract: Introduction: Diet quality indices incorporate nutritional criteria and recommendations based on food intake, eating habits, and cultural environment to characterize the diet of individuals and populations. The diet quality of athletes with disabilities needs better characterization and definition of instruments application. Additionally, athletes with disabilities need specialized monitoring in the field of nutrition. Objective: To propose a nutritional care manual for Paralympic athletes and evaluate the diet quality of Paralympic athletes and associate it with sociodemographic and behavioral variables of athletes. Methods: A cross-sectional and observational study evaluated 101 athletes with disabilities from 13 Paralympic sports in the Federal District who compete at regional, national, and international levels. Data collection was carried out over twelve months (a competitive cycle). Athletes of all modalities were divided into four groups. The interviews with each group lasted three months. The 24-hour recalls (R24h) were performed on non-consecutive days, two with all athletes, and half of them performed two more R24h. Diet quality was assessed using the Revised Brazilian Healthy Eating Index (BHEI-R) and the Global Diet Quality Score (GDQS). Diet quality was calculated using the first 24-hour recall (Rec1) and the usual intake using the Multiple Source Method for correction of intrapersonal variability. Sociodemographic data obtained from the athletes were education, sex, age, socioeconomic level, nutritional support, sports classification level, and scholarship program. The Wilcoxon test and the Spearman rho test was used to compare and correlate, respectively, usual intake and Rec1. For the construction of the nutritional care, the manual we adapted according to the American Dietetic Association. Results: In Chapter 3, we present the construction of the nutrition care manual for the Paralympic athlete, adaptations were necessary in the stages of care, especially in the anamnesis stage, in order to capture the specifics of the condition of each athlete with a disability. In chapter 4, the article directed to the quality of the diet. The median values (interquartile range, IQR) for the BHEI-R were obtained from the Rec1 and usual intake, 60.3(11.1) and 80.7(6.2), respectively. For the GDQS the median (IQR) was 19.5 (6.5) and 18.3 (2.6), respectively. Most athletes reached the classification “needs modification” and “moderate risk”, for BHEI-R and GDQS, respectively. Spearman's correlation between GDQS and BHEI-R for usual intake (ρ = 0.579; p <0.0001) and Rec1 (ρ = 0.484; p <0.0001) were positive and significant. The group that received nutritional support scored higher compared to the group that did not receive nutritional support. Conclusion: The diet quality of athletes with disabilities in the Federal District was of intermediate classification for both indices, and those who had nutritional monitoring had higher scores. In addition, the structuring of the nutrition care manual for Paralympic athletes will support future nutritionists who want to work in the field of Paralympic sport.
