Abstract:	Recent biotechnological, scientific changes and the way of providing qualified and safe health care have raised new and challenging questions related to the assessment and clinical management of patients in the perioperative period. This study aimed to evaluate and systematize the practices of a Perioperative Medicine Service with a view to the development of an Electronic Perioperative Health Record Prototype parameterized to normalization, maturity models and Evidence-Based Practices. Through a descriptive-exploratory case study, it promoted an unstructured literature review, where the themes Evidence-Based Medicine, Evidence-Based Practice, Perioperative Medicine, Electronic Health Records and their maturity and standardized models were explored, which it was followed by procedures for consulting historical documents and field research using direct observation, records in a field diary and interviews, generic meetings and Joint Application Design Meetings. The research results were categorized in a presentation by the Perioperative Medicine Service and its work processes; materialization of Evidence-Based Medicine and Evidence-Based Practice and its new models of knowledge in the perioperative context; risk multi-stratification systems and perioperative assessment and management flowcharts; institutional computer system, institutional electronic health record and its outpatient interface. The analysis of these results made it possible to demonstrate the complexity of the perioperative assessment and management; the identification of limitations involving electronic medical records from hospitals and outpatient clinics in relation to normalization parameters, their non-conformities to a maturity model and to modern guidelines in perioperative practices; the characterization of the work of the Service under study, its perioperative clinical care practices and the assessment of its compliance with Perioperative Medicine and the recent guidelines for quality, safety, evaluation and perioperative management; identify that the structuring of an optimized perioperative evaluation and clinical management system, technically and scientifically based and using a model represented by macrofluxogram, work process diagrams, perioperative functionality diagrams and process maps perioperative work models modeled in Business Process Model and Notation are the relevant requirements and should be elicited in the prototyping of a Perioperative Electronic Health Record profiled to maturity models and to Perioperative Medicine.
