Abstract:	Facial defects may be generated by traumas, congenital anomalies and resections for treating pathologies. Whatever the cause, the loss of facial substance promotes functional and aesthetic losses, psychological impact and restricts social relationships. The rehabilitation of facial defects can be realized by surgery and/or by prostheses. Facial prostheses supported by extraoral implants are treatment options that allow good symmetry and similarity of color and anatomical details, being an alternative option after surgical treatment failure. The aims of this study are to evaluate survival rates and complications related to implant surgery with auricular prostheses rehabilitation purpose, through a literature review; analyze microbiological differences in prosthesis implant-abutment interface, considering the effects of cleaning, and describe the microbiological profile of two patients rehabilitated by auricular prostheses supported by implants. A literature review, involving controlled vocabulary and free terms, was conducted in the PubMed database. Then an in vitro study was conducted on different platforms and connections to establish microbiological differences of cleaning plataforms with chlorhexidine solution. Finally, the colonization differences and the management of and two cases rehabilitated with auricular prostheses, which had soft tissue reactions, were described. Loss of implants and reactions of skin/mucosa were the most cited complications. Survival rates ranged around 90-100%, depending on factors such as the installation site of the implant and previous radiation. In the laboratory, hygienic procedures with alcoholic solution of chlorhexidine at 2% have been proven effective, with no differences between the systems evaluated or whether the parts were held together or disconnected before cleaning. The patients evaluated showed differences in the clinical aspects, microbiological profile and antibiotic sensitivity. It was concluded that the retention by implants is reliable and safe for craniofacial prosthetic reconstruction, but careful hygiene, aftercare and regular maintenance are important to minimize complications. Chlorhexidine has a value if used as a topic antiseptic, both in laboratory and clinical analysis, but retention factors; such transcutaneous components can reduce the response to the antiseptic.
